Eliot's Homeless Division is seeking an individual with strong engagement, support planning/implementation and organizational skills to take on the role of Director of Complex Care Nursing.
Responsibilities:
Supervise nurses and nursing
Develop and oversee a RN/LPN practicum experience for Homeless Services programs.
Work alongside Eliot’s recruitment team to support RN/LPN recruitment and retention across the organization.
Provide direct nursing support to assigned individuals including the provision of harm reduction counseling/education and appropriate supplies to mitigate risk of overdose.
Participate in on call rotation for Safe Haven and Boston Based programming.
Support Master Lease team in all aspects of housing placement, transition and orientation.
Provide direct assessment and support to assigned individuals in conjunction with the Medical Director of Homeless Services including but not limited to: medication management linkage, referrals, specialized case management and systems advocacy.
Respond to crisis/high risk situations collaboratively with leadership and field personnel.
Complete CSP-HI assessments and documentation as needed/assigned.
Complete required HMIS documentation and reports in a timely fashion
Meet with high risk clients on the street within master lease team, in treatment programs, hospitals and houses of correction in order to assess, establish and maintain continuity of care.
Participate in relevant care team meetings (internal and external) to ensure appropriate nursing involvement/intervention as it relates to client care.
Maintain progress notes and all documentation related to client contact and care coordination.
Qualifications:
Licensed registered nurse in Massachusetts with a minimum of a bachelor’s degree in nursing and at least 5 years’ experience working with adults with complex medical and/or behavioral health diagnoses.
Must possess working knowledge of psychiatric medications, psychiatric and medical treatments, and population health management quality and performance metrics.
Certified Addiction Registered Nurse (CARN) preferred.
Demonstrated skill working with patients experiencing homelessness with severe trauma, mental health diagnoses and substance use disorders.
Able to work autonomously in a clinical setting or in a street-based setting.
Must have a valid driver’s license and reliable transportation to and from work and for the transportation of clients as needed.
