UPMC Licensed Practical Nurse, OP Lemoyne Transition of Care in Lemoyne, Pennsylvania
Are you an experienced Licensed Practical Nurse (LPN) in the Lemoyne, PA area looking for a position with the opportunity to work from home after 6 months? You must live within 90 minutes of the Lemoyne, PA office to be considered for this position.
UPMC Centralized Call Center is looking to add a Licensed Practical Nurse to their team to complete transition of care calls to PHMG patients post discharge. This position will require in-person training and work from the office for the first 6 months. After that, you could have the opportunity to work from home. Past experience as an LPN in primary care/hospital care is required for the position.
Schedule: This is a full time, day shift position with a start time of either 8:15a or 8:30a.

Purpose:
Provides professional nursing care to office patients under supervision of patient care providers.
Responsibilities:
Draw blood.
Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health).
Administer medications including oral, SQ, IM and IV drip.
Assist physicians with examinations, diagnostic procedures and treatments.
Assess patient's general condition and take vital signs and weight.
Comply with all policies and procedures.
Perform IV therapy under direction of Registered Nurse.
Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record.
Maintain strict confidentiality related to medical records and other data.
Promote wellness by providing patient education materials, communicating physician advice/instructions.
Graduate of an approved LPN school of nursing. One year clinical experience preferred.
Licensure, Certifications, and Clearances:
CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
Licensed Practical Nurse (LPN)
Act 34 with renewal *Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
