Dartmouth Health Addiction Recovery Coach - Psychiatry in Lebanon, New Hampshire

Overview

TheAddiction Recovery Coachengages with patients and provides recovery coaching services to qualifying patients on the inpatient medical and surgical services during hospitalization and post discharge. They use shared experience, motivational interviewing, and practical problem solving to provide non-clinical services to qualifying D-H patients affected by substance use disorders and or substance use crisis events.

They act as a liaison between patient and institutional/or community resources to help patient engage in appropriate substance use treatment, peer recovery, and/or harm reduction services to promote successful linkage to treatment services and help patients identify and access needed resources and recovery supports.

This is a full-time, 40-hour/week benefitted day-shift position working Monday - Friday. Weekends and the six holidays observed by DHMC are not required.

Responsibilities

  • Meets with patients at request of patient and/or select D-H department providers, using training and shared experience to engage patients and inform them about opportunities for support.

  • Educates patients and/or their caregivers about treatment, recovery and harm reduction options, and offers to assist with accessing these services post discharge.

  • Educates patients and/or their caregivers about overdose reversal medication and works with D-H providers to dispense these medications when appropriate.

  • Provides follow-up calls/meetings with patient for 1-3 months post discharge, and ongoing if needed, providing referrals and continuing support for assessment, treatment, recovery, and harm reduction services.

  • Assist individuals to identify and develop natural community supports in order to foster independence and community integration.

  • Provides information and referrals regarding recovery resources in the community.

  • Maintains professional boundaries at all times, accurately represents the role of Recovery Coach and encourages patient to seek clinical and/or medical advice from their appropriate provider.

  • Attends meetings and/or trainings as required or encouraged by departmental leaders.

  • Maintains non-clinical documentation of coaching contacts as required by departmental leaders

  • Willingness to address stigma or the perception of stigma in the medical setting.

  • Help to facilitate collaboration and communication between patients and medical staff.

  • Maintains patient confidentiality and complies with HIPAA regulations.

  • Performs other duties as required or assigned.

Qualifications

  • Undergraduate Degree preferred, but will consider persons with High School Diploma/GED or professional experience in field.

  • Be a person who identifies as in recovery from a substance use disorder and be willing to share details of his or her personal recovery experience

  • Experience relating to persons with substance use disorders needed; empathic and non-judgmental approach essential.

  • Completed Training as Recovery Coach, including ethics training

  • Certification as a Recovery Support Worker (CRSW) required at time of hire or within 1 year of employment

  • Two years of continuous sobriety at time of hire.

  • Thorough understanding of stressors impacting patents in recovery from opiate use disorders

  • Able to maintain patient confidentiality

  • Basic computer literacy skills

  • Strong interpersonal, communication, and customer service skills.

  • Developed problem-solving skills

  • Ability to develop professional working relationships with community partners and organizations
