We seek a dedicated Case Manager to join our Behavioral/Mental Health Program. You'll assess client needs, develop care plans, and advocate for clients to ensure they receive the best care.
Key Responsibilities:
Assess clients' mental health needs and develop and implement personalized care plans.
Coordinate services between providers and agencies.
Monitor client progress, adjust care plans, and assist with therapy sessions.
Advocate for clients to access resources.
Maintain accurate client records.
Provide crisis intervention and support.
Educate clients and families about mental health.
Offer specialized support to LGBTQ patients.
Collaborate with multidisciplinary teams.
Minimum Qualifications:
Bilingual Spanish and English.
Knowledge of community services and resources.
Understanding of mental health conditions and LGBTQ issues.
Excellent communication and organizational skills.
Experience with electronic health records (EHR), preferably Athena.
Compassionate, empathetic, and strong advocacy skills.
Preferred Qualifications:
Degree in social work, psychology, counseling, or related field.
Working Conditions:
Office environment with occasional travel.
May require evening or weekend hours.
