Lansing Area AIDS Network Medical Case Manager in Lansing, Michigan
POSITION: Medical Case Manager REPORTS TO: Care Program Managers SALARY: $45,000 JOB SUMMARY: Under the supervision of the Care Program Manager(s), this person is responsible for the provision of medical case management oversight, including adherence to medical protocols, risk reduction and health education services, and outreach to targeted populations, i.e. bringing clients out of care into care. The goal of the Medical Case Manager is to help individuals living with HIV to access primary medical care and medications, identify and remove barriers to medical care, and ensure adherence to prescribed medical treatment. The Medical Case Manager meets regularly with the care team to monitor program objectives and other issues pertinent to the successful completion of various goals. The position is also responsible for attendance at regular staff meetings. LICENSES AND CERTIFICATIONS A valid Michigan Vehicle Operator's License and Michigan Department of Health and Human Services Certification as a Case Manager (or ability to obtain within three months of hire or as MDCH training schedule allows). EDUCATION At minimum, a Bachelor's Degree in Social Work or equivalent; medical training or work in the medical field and experience with HIV/AIDS will be helpful. EXPERIENCE Experience working with chronically ill, diverse populations, experience in HIV/AIDS or the willingness to become facile in all aspects of HIV/AIDS.
