Sensory Tool House, LLC Assistive Technology Case Manager in Lacey, Washington

Job Title: Assistive Technology Case Manager Employment Type: Full-time (35-40 hours/week)

Position Overview: We are seeking two full-time Case Managers. The Case Manager will play a vital role in supporting participants with assistive technology needs from program entry through completion. This position will require travel to Lewis, Mason, and Grays Harbor Counties. Office location is in Lacey, Washington.

Key Responsibilities

Participant Support & Case Management

  • Conduct eligibility screenings, functional capacity evaluations, and needs assessments to match participants with Assistive Technology that will further their ability to participate in education, employment, and/or community access.
  • Collaborate with community partners to identify eligible individuals and ensure seamless service delivery.
  • Manage a caseload of eligible participants, providing personalized, participant-centered support.
  • Recommend and coordinate tailored AT solutions based on individual needs, goals, and strengths.
  • Support coordination for participation in digital literacy training plans for participants.

Training & Technical Assistance

  • Provide one-on-one training sessions to ensure participants gain proficiency with AT devices.
  • Create accessible training guides for AT devices provided to participants.
  • Deliver ongoing technical support and follow-up sessions to address challenges and adapt solutions as needed.

Procurement & Coordination

  • Coordinate the procurement of AT devices with the purchasing department, ensuring timely and accurate order fulfillment.
  • Oversee installation of AT devices within the timeline.

Program Evaluation & Reporting

  • Collect and analyze participant data to assess program impact and effectiveness.
  • Administer questionnaires and provide feedback to improve services and outcomes.
  • Participate in preparing reports for stakeholders, demonstrating program success.

Compliance & Collaboration

  • Ensure compliance with federal and state disability laws and ADA guidelines.
  • Engage with subject matter experts and community partners to enhance services and training materials.

Required Qualifications

  • Valid driver's license, reliable vehicle, and proof of car insurance (required).
  • Current CPR Certification by time of hire (Required)
  • Ability to pass a criminal background check upon offer of employment
  • Strong knowledge of assistive technology and ADA compliance.
  • Experience in case management
  • Ability to keep detailed records and communicate with a diverse population
  • Ability to travel to various locations for participant screenings and service delivery

Desired Qualifications or Experience

Licensed Occupational Therapist (OT), Licensed Physical Therapist (PT), Certified Disability Management Specialist (CDMS), Assistive Technology Specialist (AT Specialist) or other relevant experience or licensure

Certified Ergonomic Assessment Specialist (CEAS)

In-depth knowledge of assistive technology tools and their applications across various settings (education, employment, and community access).
