WorkSource Oregon Housing Navigator in LA GRANDE, Oregon

The Housing Navigator will require a compassionate individual dedicated to supporting and empowering individuals facing homelessness within our four-county service area. The Housing Navigator will respond to referrals to support houseless individuals with low incomes, engage in homeless outreach, and provide access/referrals to various supportive services. Requirements: * College education, with emphasis in social work or human services, is desired, or comparable education and experience. * High School diploma or equivalent. * Valid driver\'s license for traveling to each of the counties. * Family Development Specialist, ROMA (Results Oriented Management Accountability), Community Health Care Worker Training, ServicePoint HMIS (Homeless Management Information System). * One year experience working in social service programs with case management experience preferred. Experience working with low-income people is an asset. * Pass employer pre-employment drug test. * Pass employer criminal history background check. Duties: * Coordinate with the Rent Well Providers, DHS, and other agencies to address the needs of households. * Perform screenings, intakes, and assessments via Coordinated Entry System referrals. * Document services in HMIS/ServicePoint. * Direct clients to specialized services and resources. * Promote permanent housing opportunities by engaging and developing relationships with property owners/managers. * Facilitate access to temporary housing and aid in transition to permanent housing. * Handle referrals from outreach subcommittees and provide linkage to services. * Manage and maintain a revolving caseload. * Assess clients to determine appropriate levels of service on a case-by-case basis. * Refer clients to appropriate agencies for specialized services and/or counseling. Provide and document individual/group counseling and referral services when appropriate. * Assist with resource eligibility and applications, housing location, and securing move-in funding. * Develop action and stabilization plans with clients. Guide clients through application processes, lease agreements, utility setup, and rent payment systems. * Consult with other staff, agencies, employers, property owners/managers, hotel/motel operators, and other service providers. * Offer ongoing support and crisis response. * Work in various settings, including outdoors and within the community. * Update changes in income and household composition. * Respond to specific population referrals, supporting individuals with disabling conditions at risk of homelessness. * Assist in developing immediate solutions to emergency problems and expediting delivery of needed services. * Maintain ongoing engagement and support. * Provide office back-up such as answering multi-line phone, greeting walk-in clients, etc., as needed. * Travel throughout region, to Salem, and other areas, periodically for meetings and training. Will involve overnight stays. * Other duties as assigned by supervisor or agency leadership.
