Care Coordinator, Perinatal Center

Full Time, 80 Hours Per Pay Period, Day Shift

For more than 25 years, we’ve provided comprehensive high-risk pregnancy services and delivered over 26,000 high-risk babies at Fort Sanders Regional Medical Center. We offer a range of care services for higher-risk pregnancies, including mothers over age 35, multiples, complications, birth defects, issues such as diabetes or hypertension, or pregnancies after infertility.

Position Summary:

Responsible for coordinating the care of patients across the continuum of care; facilitating the achievement of quality, clinical, and cost outcomes; negotiating, processing, and coordinating services and resources needed by the patient and family. This is achieved by a collaborative process which includes assessing, planning, implementing, monitoring, and evaluating options and services to meet the patient and family health care needs.

Responsibilities

  • Identifies patient’s physical, psychological, and learning through consultation with the physician, nursing staff, and other healthcare providers as well as through review of the medical record.

  • Assesses and makes recommendations regarding level of care placement based upon patient’s clinical status, home situation, and insurance benefits/personal resources.

  • Possesses sound knowledge base regarding Intensity of Service/Severity of Illness.

  • Assesses to identify opportunities for intervention which could include:

  • Over-utilization of services

  • Under-utilization of services

  • Lack of education regarding disease process

  • Lack of an established treatment plan with specific goals

  • Assists with patient appointments at necessary facilities prior to delivery.

  • Improve continuity of care between all physicians involved in care of patient.

  • Update referring physician office of current patient status throughout pregnancy and following delivery.

  • Work closely with all referring physician offices to ensure all necessary information is transferred back and forth in an appropriate time span.

  • Connects patients to relevant community resources to increase patient health, awareness, well-being and satisfaction while helping reduce the cost of care.

  • Be primary contact for patient who has questions or is in need of help.

  • Participation in transdisciplinary team meetings to discuss and plan patient and family needs and discharge planning to the appropriate level of care.

  • Provides critical analysis of situations which present barriers to achievement of goal.

  • Participation in the development of carepaths, protocols, and outcomes measures.

  • Collaborates with patient, family, and healthcare team to produce optimum patient outcomes in a cost effective and efficient manner.

  • Proactively develops and supports methodologies that can positively impact patient care outcomes, cost utilization and staff productivity.

  • Develops clinical knowledge base to assist in the development of protocols/pathways/outcomes which will support the continuum of care model in a cost-effective manner.

  • Continually monitors and reports trends involving agreed upon quality care issues.

  • Acquires an understanding of the coding system and works collaboratively with involved team members to support and improve documentation as it relates to coding more accurately.

  • Conducts Utilization Review for identified patients within assigned service areas.

  • Collects data concurrently for identifiable outcomes statistics as requested. Evaluates the effectiveness of the Care Coordination model for identified service area, considering benefits to patients, healthcare team and the institution.

  • Review of medical record and discussions with health care team occur daily to track adherence to specific pathway or the plan of care. Variances are investigated and the plan is adjusted based upon assessment and team collaborative efforts.

  • Promotes education and discharge planning with members of the health care team for optimal functional status and independence.

  • Facilitates referrals to the appropriate entity based upon identified needs in collaboration with the health care team.

  • Continually strives to improve quality care in a cost-efficient manner using creative problem-solving strategies with identified health care team members.

  • Demonstrates effective communication skills. Demonstrates leadership with healthcare team members to facilitate maximum patient outcomes.

  • Continually evaluates and participates in process improvement initiatives that relate to increased quality and efficiency, decreased LOS, and cost reduction.

  • Consistently evaluates the patient’s responses to interventions and monitors their progress through discharge.

  • Participates with the DRG team in the development of quality outcome measurements for identified patient populations.

  • Consistently seeks feedback related to the role in order to enhance the care coordination initiative.

  • Determines the reasons for a static or regressive situation related to the patient’s status and proactively engages in a collaborative effort to implement adjustments for improved outcomes.

  • Seeks feedback and adjusts work habits and behaviors accordingly based upon specific feedback

  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested.

  • Performs other duties as assigned.

Qualifications

Minimum Education:

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

Minimum Experience:

Must have a minimum of three (3) years of clinical experience working within a women’s services center.

Licensure Requirement:

Current license in good standing with the State of Tennessee, (MSSW/LCSW)
