We are now offering a sign-on bonus of \$1500 upon hire. Eligible for \$1,000 after your first 60 days and \$500 eligible to be received after 6months.
Job Preview at a Glance:
The Case Manager serves as a central support within the interdisciplinary team. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency.
Pay Rate: Starts \$17.50 hourly
Responsibilities:
Intake screenings (Completing intake paperwork and referrals)
Completing annual behavioral health assessments, including service planning and support planning
Facilitating CFTs and ARTs
Facilitating groups
Identifying gaps in care for our members
Completing SMI and SED evaluation
