Sevita Case Manager Adult Health Services in Kentucky

Just Family Center, a part of the Sevita family, provides home and community-based services including day programs for elders with complex medical or rehabilitative needs, attendant care and case management. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

SUMMARY

Meets and works with individuals to gather information, assess each client/individual's needs and assess the cost of care. May prepare a plan of care, and identify and implement services to meet an individual's needs.

ESSENTIAL JOB FUNCTIONS

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:

  • Provides case management services including intake, assessment, crisis intervention, advocacy, referral, and monitoring of families.

  • Gathers social history to assess family strengths and weaknesses, and existing problems.

  • Conducts home visits and office base services, and accompanies individuals being served as needed.

  • Coordinates care with outreach workers, health educators, nutritionists and other service providers to prevent service duplication and to ensure the individual receives high quality care.

  • Identifies and procures group services appropriate for the individual to maximize the individual’s ability to live outside an institution. Plans and assists in obtaining services from third-party service providers.

  • Develops a plan of care for each individual to include services such as advocacy/case management, independent living skills, training, peer support, physical therapy, cognitive therapy, etc.

  • Monitors the delivery of services in coordination with the individual and third-party service providers.

  • Assists individuals with related matters including individuals’ eligibility for benefits through Medicaid, Medicare, Social Security and private insurance.

  • Maintains up-to-date case records with the status of case activity, including progress notes on all contacts.

  • Performs other related duties and activities as required.

Minimum Knowledge and Skills required by the Job

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:

Education and Experience:

  • Associate's degree in Human Services with one year of related experience preferred.

  • An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

  • Knowledge of case management.

  • Experience working with Disabled Adults

Certificates, Licenses, and Registrations:

  • Current driver's license, car registration and auto insurance if providing transportation to individuals receiving services.

  • Current CPR/First Aid Certification if required by state/program.

Other Requirements:

  • Travel as needed

Physical Requirements:

  • Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.
