UH Lakewood Medical Center is located between Kansas City and Lee’s Summit, Missouri, offering residents of Eastern Jackson County access to a modern community hospital with a continuum of outpatient services designed to meet the needs of families. UH Lakewood specializes in high-risk maternal and child care, and serves as one of the few Kansas City metro area institutions accredited by The Joint Commission in Orthopedics and Palliative Care. It also is home to a 188-bed, fully accredited long-term care center.

UH Behavioral Health is the KC metro area’s largest, most comprehensive provider of mental health services. The Crossroads Building , located in Kansas City’s vibrant Crossroads Arts District, is its epicenter, providing a broad list of outpatient services. We also offer psychiatric inpatient care, and we deliver therapeutic services through our community outreach clinics. We are a leader in the Trauma-Informed Care Movement, and provide training, coaching and consulting to outside organizations.

Job Description

The Community Health Worker is a frontline public health worker who performs a variety of duties to engage, support, advocate, and connect community members to needed and necessary resources to support good health and wellness. The Community Health Worker serves as a liaison or link between health and social services and the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.

This position is grant funded, and the duration of the assignment will remain as long as we receive the grant.

Minimum Requirements

  • Associates degree in community health, social work, health sciences or any related community health worker program. Candidates with a high school diploma or GED and relevant two (2) years of work experiences may still be considered.

  • Understand issues related to abuse, neglect, and criminal activity that may be reportable under law and regulation according to organizational policy.

  • Interpersonal communications, group process and facilitation skills.

  • Excellent oral and written communications skills.

  • Demonstrates ability to present information to large audiences.

  • Demonstrates computer use ability in word processing, spreadsheet, database, and presentation software.

  • Demonstrates ability to organize work, set priorities, meet critical deadlines and follow up on assignments with a minimal amount of direction.

  • Demonstrates ability to provide care coordination, which may include but not be limited to facilitating care transitions, supporting the completion of referrals, and providing or confirming appropriate follow-up.

  • Demonstrates ability to apply multiple techniques for helping people understand and feel empowered to address health risks for themselves, their family members, or their communities. (Examples may include informal counseling, motivational interviewing, active listening, harm reduction, community-based participatory research, group work, policy change, and other strategies)

Preferred Qualifications

  • Bachelor’s Degree in social work, public health, health sciences or other closely related field.

  • Two or more years of progressively responsible experience as a community health worker.

  • Bi-lingual (English and Spanish proficiency)

  • Experience with community organizing and/or mobilization

  • Knowledge of local population demographics, assets, and needs

  • Knowledge of local community health disparities and resources to address those disparities
