Responsibilities:
Triage all incoming phone calls for Hospice Team.
Assist in referral management by gathering required information, updating referral tracker and working closely with the Director or Nursing.
Assist in ordering Durable Medical Equipment.
Work closely with all hospice team members to ensure compliance with required patient documentation.
Responsible for keeping new admission paperwork and supplies prepared for Hospice Nurses.
Compiles and organizes information for Interdisciplinary Team meetings,
Complete referral/intake forms and route them appropriately for admission approval.
Ability to work in a fast-paced environment and be flexible.
Perform other duties as directed.
Requirements
Experience in a Hospice setting preferred.
Quality Assurance experience preferred.
Must have excellent computer skill - Excel, word
