GENERAL OVERVIEW

Abstracts clinical data from medical records in accordance with regulatory and administrative reporting requirements. Enters abstracted data into the Core Measures database and provides relevant internal and external reporting to support clinical care, performance improvement, outcomes management, and relevant regulatory compliance for the organization.

ESSENTIAL RESPONSIBILITIES

• Retrieves required data elements from hospital mainframe as needed. Enters data into the core measure database. Maintains productivity in data entry in order to maintain program compliance with external reporting deadlines.

• Maintains proficiency and accuracy in abstracting, achieving and maintaining a minimum of 95% accuracy on quality reviews in order to ensure high quality of information and database integrity.

• Adapts to change and stays informed on revisions of abstraction guidelines.

• Reviews medical records as needed and maintains control and security of medical records. Maintains confidentiality of relevant information.

• Serves as a resource person to interpret and facilitate use of system data and information.

• Integrates continuous quality improvement principles into all aspects of job responsibility.

QUALIFICATIONS

Minimum

• High School diploma or equivalent plus additional course work to include medical terminology and basic anatomy & physiology required

• Prior job-related work experience of at least 1 year is required

• Prior healthcare related work experience is required

• Basic PC skills required

• Must pass a medical terminology test with a minimum score of 85% within a prescribed timeframe

Preferred

• Abstracting or medical records experience is preferred

• Database experience is preferred

• Registered Health Information Technician (RHIT) is preferred
