Shift Schedule : Monday – Friday 7:30am – 3:30pm or 8:00am-4:00pm

Salary: Salary pay range is min. $17.52 to a max. $22.82 per hr. based on education & experience

    Essential Job Functions Overview:

    With general supervision, works from a trauma informed care perspective, based on a psychiatric rehabilitative approach to provide advocacy, linkage, and coordination of services for an assigned caseload. Special emphasis is given to services that enable recipients to attain and remain in permanent and independent community housing. Individual should strive to create a healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve by practicing safe, respectful communication as well as respecting individuals’ boundaries and differences.

    Essential Job Functions:

    · Conduct a comprehensive assessment of needs and develop an individualized support plan.

    · Effectively support individuals through skills and practices including but not limited to motivational interviewing suicide prevention, risk screening, trauma-informed care and person-centered planning.

    · Assist residents with identifying and achieving person-centered goals and recovery

    · Monitor consumer wellness and ensure well-coordinated care among all providers

    · Maintain accurate documentation for all services according to established policies and procedures and submit required reports

    · Assist residents with securing safe, affordable, permanent housing

    · Assist residents in negotiating with landlords, vendors, and service providers

    · Assess needs and solicit preferences for residents, make appointments, and inspect housing

    · May purchase supplies and physically help with moving, setting up, and maintaining housing

    · Conduct monthly in-home visits and update support plans every 90 days

    · Develop and maintain professional relationships with supervisors, colleagues and community services

    · Attend county-specific meetings to ensure timely processing of referrals

    · May provide more comprehensive case management services as needed

    Education and Experience:

    High School Diploma with two (2) years of relevant experience or an associate degree in human services. Valid NYS Drivers License (as driving is an essential function of this position)

    Knowledge:

    Thorough knowledge of community services within service area; general knowledge and understanding of mental illnesses, psychiatric rehabilitation model, and related issues; skills for independent living. Has a basic understanding of the importance of working from a trauma sensitive perspective.

    Skills and Abilities:

    Use of contemporary office equipment, particularly a computer with word processing, database and report generating software; to communicate effectively with diverse individuals; to listen, understand, and appreciate the experiences of clients; to persuade, negotiate, and instruct; to manage and resolve conflicts; to provide positive role modeling; to inspire respect, confidence, and trust in clients and staff; to respect and maintain appropriate confidentialities; to effectively encourage clients toward greater independence and self-sufficiency; to perceive and describe changes in behavior; to seek, accept, and learn from peer and supervisor feedback; to organize time effectively; to plan and implement strategies consistent with client needs and overall organization goals, objectives, and standards.

    Physical Requirements and Working Conditions:

    Significant amount of driving and visiting clients; general apartment settings; general office environment. Significant amount of purchasing supplies, assisting with moving of furniture, relocation, and household maintenance.
