JOB REQUIREMENTS: The Marshfield Medical Center-Dickinson seeks a full-time Social Worker to join their team of healthcare professionals in Iron Mountain, Michigan. The ideal person for this position has a compassionate nature able to provide assessment and psychosocial support to patients, families and caregivers, collaboratively working toward objectives of achieving the best possible quality of life. WHAT WE OFFER. \$51,064 - \$79,476 annual salary based on experience. Health, dental and vision insurance; prescription drug coverage. Short & long-term disability, and life insurance. Paid time off (sick, vacation). Full-time Monday-Friday position (36 hours per week) with start times of 7:30am or 8am. WHAT YOU WILL DO. Provide direct social work services to patients and families, addressing social, environmental, financial, and emotional issues. Assist patients and families in coping with stress and anxiety related to medical conditions. Facilitate patient placement in nursing homes and home health, and seek options for financial aid, chore services, transportation, and nutrition programs. Develop and coordinate discharge plans, referring patients to appropriate social services and community resources. Consult with hospital staff, community agencies, and medical staff to resolve patient problems and interpret social/psychological, emotional and family issues. Coordinate the hospital swing bed program and monitor patient eligibility; inform physicians of potential candidates; inform Quality Improvement Committee on program use and any roadblocks. Lead and direct special projects as needed. WHAT WE ARE LOOKING FOR. Bachelor\'s Degree in Social Work or a related degree required. Certification as a Social Worker in the State of Michigan required. Experience in an acute care hospital or healthcare agency preferred. Knowledge and understanding of community health, welfare, and social agencies. Understanding of medical terminology. Exceptional customer service and communication skills with a professional demeanor that ensures trust, competence and compassion amongst patients and team members. Ability to organize and prioritize multiple tasks simultaneously. Ability to successfully pass a pre-hire drug screening and background check. SOUND LIKE YOU?
