Join Community

Community Health Network was created by our neighbors, for our neighbors. Over 60 years later, “community” is still the heart of our organization. It means providing our neighbors with the best care possible, backed by state-of-the-art technology. It means getting involved in the communities we serve through volunteer opportunities and benefits initiatives. It means ensuring our dedicated caregivers can learn and grow to stay at the top of their fields and to better serve our patients. And above all, it means exceptional care, simply delivered — and we couldn’t do it without you.

Make a Difference

Clinical Podiatry has a full-time Ambulatory LPN position available. The Ambulatory LPN will follow the Doctor as their extender doing nailcare. The practice locations are: 7400 Shadeland Ave. Suite 105 in Indianapolis, 10122 E. 10th St. Suite 210 in Indianapolis, and 1622 Madison Ave. in Anderson, IN. The Ambulatory LPN will assist the provider of a health care site with facilitation of patient visits and procedures including: wound debridement, dressings, wound measurements, documentation, routine laboratory tests, referrals, and quality outcomes for the population of the practice. Helps cover back-office patient care and administrative responsibilities in the event of clinician illness, approved time-off, jury duty, funeral time, times of high census, other growth needs determined/identified by practice management team.
