Pharmacy Care Representative

Through our Center for Connected Care, we provide a full continuum of care so that patients can recover from illnesses or injuries in the very best location for their individual needs. As a Pharmacy Care Representative, you will work in the home infusion pharmacy.

A caregiver in this position works 8:30am - 5:00pm or 9:30am - 6:00pm, every sixth/seventh weekend, and rotating on-call shifts.

A caregiver who excels in this role will:

  • Complete important data entry tasks such as patient demographic information and IV prescriptions.

  • Work closely with the pharmacy team to ensure deliveries, supplies and equipment are managed appropriately and in accordance with patient needs and department policy.

  • Serve as primary point of contact regarding deliveries and home infusion related issues.

  • Enter and maintain computer data including patient demographics such as allergies, height and weight.

  • Maintain a master calendar for patient deliveries.

  • Collaborate with the delivery supervisor about the delivery schedule.

  • Maintain patient's supply template to reflect IV therapy needs.

  • Enter prescriptions as needed to support IV therapy needs (i.e. flushes).

  • Maintain the home medical equipment schedule for annual maintenance.

  • Assist the HME Equipment and Inventory Representative (formerly Equipment PSR) tracking HME for return.

  • Obtain each patient's home inventory with each delivery to determine supply needs.

  • Ensure documentation of inventory meets payer guidelines.

  • Recruit, initiate protocol and train patients on MyChart system.

Minimum qualifications for the ideal future caregiver include:

  • A high school diploma or GED.

  • Certified Pharmacy Technician (CPhT) verified through the Pharmacy Technician Certification Board (PTCB).

  • Registration as a Certified Pharmacy Technician with the Ohio Board of Pharmacy within 90 days.

  • Two years of general office experience, pharmacy experience or experience with insurance processing or appointment scheduling.

  • Working knowledge of computerized appointment scheduling systems, insurance processing or other related office functions.

  • Knowledge of medical terminology.

  • Computer skills and proficiency with Microsoft products, email and windows applications.

  • An associate's degree may substitute one year of experience.

  • A bachelor's degree may substitute two years of experience.

  • A job-related training program may substitute some of the experience.

Preferred qualifications for the ideal future caregiver include:

  • Experience in the pharmacy field.

  • Experience with sterile compounding.

Physical Requirements:

  • Dexterity sufficient to operate a PC and other office equipment. Ability to lift and transport up to 25 pounds. May require extended periods of standing, walking or sitting. Good visual acuity through normal or corrected vision.

  • Follows standard precautions using personal protective equipment as required.
