Cleveland Clinic Community Paramedic in Independence, Ohio

Community Paramedics support existing health services in partnership with other health professionals including community organizations. We will have a few mandatory meetings at the BOC (Business Operation Center). While working in the Community Paramedic role, some visits will take place at patient's homes. We are looking for someone who is flexible and can multitask.

Each Community Paramedic has a primary territory assigned . Based on what referrals come in for the day, the scheduler will make every attempt to keep each medic in their primary territory. However, a medic may be pulled into other areas depending on the referrals that come in and which medics are working.

This Primary Assignment:

  • South East

  • South of Beachwood

  • North of Akron

  • Includes Richfield, Brecksville, Independence, Aurora, Chagrin, and Warrensville Heights.

A caregiver in this position works Mon-Fri, from 8:30am-5:00pm.

A caregiver who excels in this role will:

  • Provide home based care that supports optimal health and wellness for the patient, within the scope of practice of a paramedic (EMT-P), under the direction/supervision of a Licensed Independent Provider (LIP) and appropriate health team members. ​Perform assessments and reports findings of both in-depth and focused healthcare assessments, screenings, and examinations to the appropriate team member (s) (this may be done using tele-presence, telephone, and/or written documents). These assessments take place in a residential environment requiring the caregiver to operate a motor vehicle and practice in a variety of community settings and locations.

  • Support existing health services in partnership with other health professionals including community organizations.

  • Provide primarily supportive care and education with limited interventions.

  • When appropriate, Basic Cardiac and Advanced Life Support may be provided as outlined in existing Cleveland Clinic EMS protocols in coordination with local Emergency Medical Service providers.

  • Examine, screen, treat, and coordinate health services for patients (may include point of care laboratory testing based on paramedic's education, training, and competency):

  • Conduct/assist with post-hospital care including home safety/living condition evaluations, medication adherence/education, dressing changes, checking vital signs and reinforcement of post-discharge/ post-office visit education.

  • Observe, record, and report to the care team/LIP, patient's condition, response to medications and/or treatments and any significant findings/incidents (actual /potential - Risk Assessment).

  • Conduct patient education including chronic disease management (Diabetes, Hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), fall/injury prevention, basic nutrition, post-operative wound monitoring, and care.

  • Coordinate appointments, additional home health visits and follow-up with providers and facilities as appropriate/ordered.

  • Provide both Basic and Advanced Life Support within their scope and capabilities (equipment limitations) in collaboration with local EMS agencies.

Minimum qualifications for the ideal future caregiver include :

  • Completion of accredited paramedic training program (EMT-P).

  • Current certification as an Emergency Medical Technician - Paramedic in the State of practice.

  • Basic Cardiac Life Support (BLS) through the American Heart Association (AHA)

  • Advanced Cardiac Life Support (ACLS) through the American Heart Association (AHA)

  • Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.

  • Must have reliable transportation and proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8-175 thereafter.

  • Two years of recent clinical experience as an EMT-P

Physical Requirements:

  • Requires full body motion to assist patients and practice within a home environment.

  • Ability to move and lift patients, manual finger dexterity with good eye-hand coordination, involves extensive standing and walking.

  • MEDIUM WORK - Exerting 20-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.

  • Physical demand requirements are in excess of those for 'Light Work.'

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.

  • Access to Cleveland Clinic provided cellular phone and IPad.
