Pathfinder Services Nurse in Huntington, Indiana

Hours:Mon-Fri 8a-4p Flex (40hr)
As the Nurse at Pathfinder, you will serve as a medical case coordinator and advocate for individuals in disability programming. Your primary responsibility is planning, recommending, and implementing quality nursing care, including training and consultation on medication administration and medical practices.
Your role involves managing a caseload of individuals in two main settings:
Supervised Group Living (SGL):This residential option caters to individuals with intellectual/developmental disabilities, typically hosting five to eight people, with the setting and services adapting to each individual's needs.
Residential Habilitation and Support (RHS):An independent Medicaid Waiver residential option for individuals with intellectual/developmental disabilities, with 1-4 people per home. You will oversee wellness and health-related risk plans for RHS services with wellness coordination.
Your responsibilities align with the Indiana Nurse Practice Act, Medicaid Waiver standards, CARF standards, Indiana State Department of Health standards, and Pathfinder policies. Embracing a holistic health and wellness approach, you will help individuals achieve their life goals. Some key responsibilities include managing and coordinating healthcare needs by collaborating with healthcare professionals and the interdisciplinary team. Prepare health-related documentation for group home and supported living residents, including progress notes, high-risk plans, annual reports, quarterly reviews, and maintenance of medical charts. Youll also document medical interventions and contacts, ensuring compliance with established standards.

Requirements
Preferred Minimum Qualifications
- Current license in the State of Indiana as issued by the Indiana State Board for Registered Nurses is required.
- Demonstrated ability to support people with disabilities.
- Knowledge and effective use of electronic documentation, reporting, communication, etc.
- Ability to project a professional image while engaging with community partners, healthcare providers, and individual support teams.
- Crisis prevention, intervention, and teaching skills.
- Valid drivers license, insured vehicle, and safe driving record.
- Preferred minimum of three (3) years of experience in community health/mental health services.
- Ability to speak, read, and hear the English language.
- Essential physical skills: be able to run, squat, stoop/bend, kneel, climb stairs, lift up to 50 pounds, push and pull against potential resistance, and implement non-physical and approved physical interventions and CPR with people supported when needed
