Position Summary:

Responsible for coordination and completion of a wide variety of projects to assist in the implementation and monitoring of healthcare transformation activities.

Position Key Accountabilities:

  1. Responsible for the coordination and completion of projects.

  2. Oversees all aspects of projects.

  3. Sets deadlines, assigns responsibilities, monitors and summarizes progress of project.

  4. Prepares reports for upper management regarding status of project.

  5. Familiar with a variety of the field's concepts, practices, and procedures.

  6. Relies on experience and judgment to plan and accomplish goals.

  7. Interacts with representatives from various departments to develop, implement, and manage policies and procedures related to assigned project(s).

  8. Works within budget, negotiates service contracts, and reports on progress.

  9. Works closely with internal departments regarding strategy and collaboration.

  10. Prepares progress reports for both management and governmental agencies.

  11. Interacts with representatives from various departments to develop, implement, and manage policies and procedures related to assigned project(s).

  12. May prepare/make oral and/or visual presentations.

  13. Develops and maintains project documentation.

  14. Performs a variety of tasks.

  15. May lead and direct the work of others.

  16. Functions in a leadership capacity in the project.

  17. Manages Human Resources activities of department in regards to: recruiting and selection, hiring and termination, training, professional development, mentoring, counseling, performance evaluations, and salary planning.

  18. Uphold internal controls which provide reasonable assurance that operations are effective and efficient, assets are safeguarded, and financial information is reliable and compliant with applicable laws, regulations, policies and procedures.

  19. Responsible for coordinating, and implementing innovative programs to transition from fee-for-service reimbursement to a system based on value (e.g., Accountable Care Organizations, bundled payments, payer gain-sharing, risk-based contracts, etc.)

  20. In collaboration with Directors, manages all aspects of value-based care models, value-based structures related to quality performance, and parallel programs across all lines of business.

  21. Lead program implementation work plans and work groups across program committees, including facilitation and management of performance reporting.

  22. Manages and coordinates strategic value-based care initiatives that will drive quality performance, clinical care gap closures,clinical documentation improvement,and decrease the overall cost of care.

  23. Creates best practice workflows and process mapping for optimal delivery of care to meet measure targets.

  24. Ensure the strategic directives of the stakeholders are executed, and time-sensitive deadlines met. Works as the subject matter expert for value-based projects.

  25. Collaborates with providers, leaders, and other departments to ensure negotiated contracts can be operationalized.

  26. In coordination with business analysts, conduct risk analysis of project implementation and objectives, and propose relevant countermeasures.

  27. Other duties as assigned.

Certification/Skills:

None.

Minimum Education:

Bachelor’s degree; Master’s degree preferred.

Minimum Experience:

Three (3) years in project/program management.

Knowledge of value-based program strategies, cost-of-care initiatives, and performance analytics is preferred.

Physical Requirements:

Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.

Residency Requirement:
