Job Summary and Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered, compassionate care in coordination with our physician partners and patient care team. The ideal candidate is an experienced clinician with who can work independently and collaboratively within a team. To be successful in this role you must possess the following qualifications: A valid Texas Physical Therapist license, a Doctorate of Physical Therapy degree, previous experience working with wound care patients in an inpatient setting. Demonstrated ability to work independently and collaboratively within a team. Excellent patient assessment, communication and documentation skills. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.

What you will do in this role:

• In this role you responsible for evaluating, planning, treating, and performing interventions within the scope of practice established by law and professional code of ethics.

• You will provide purposeful and skilled therapeutic procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.

• Create individualized treatment plans and goals for rehabilitation

• You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care

• You will educate patients and families on wound care, prevention and treatment.

• Collaborate with other departments to meet the needs of the patient

• You will prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

• Participate in continuing education and professional development activities.

What qualifications you will need:

• Doctorate of Physical Therapy Degree

• Currently possess and maintain a valid Texas Physical Therapy license

• Certified Wound Care specialist certification preferred

• 1 year experience working in an inpatient setting

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
