DESCRIPTION OF DUTIES / ESSENTIAL FUNCTIONS

The Houston Health Department seeks a dynamic candidate to fill the Human Service Program Manager position within our Diabetes Awareness and Wellness Network (DAWN) program. This management position will oversee multiple DAWN sites and be responsible for overseeing day-to-day operations, leading a muti-disciplinary team of public health professionals, managing program budgets, and ensuring successful delivery of community services. This role requires strong leadership and program management skills. The selected candidate will perform the following duties along with other duties as assigned:

  1. Plan, implement, oversee and coordinate Chronic Disease and Diabetes programming at multiple DAWN sites, ensuring alignment with organizational goals and objectives.

  2. Gather and analyze data and information that identifies community needs and assists with the development of viable programs, and the evaluation of program effectiveness.

  3. Coordinate program activities, including patient education sessions, support groups, and wellness programs, to promote effective diabetes management.

  4. Oversee program budget, resources, and staffing to ensure efficient operations and optimal patient outcomes.

  5. Assist in coordinating approval for facility purchases, operations, maintenance and special projects.

  6. Facilitate and lead efforts to establish and maintain partnerships with community organizations, healthcare providers, and other stakeholders to enhance program reach and impact

  7. Serve as a liaison and collaborates with governmental and public agency representatives, community outreach and advocacy groups, and service providers.

  8. Develop and maintain active and viable Advisory Councils utilizing the DAWN Coalition and other community groups.

  9. Develop and participate in education, training marketing strategies and presentations related to DAWN services to various partners and departmental leadership.

  10. Monitor program metrics and outcomes, utilizing data-driven insights to evaluate program effectiveness and identify areas for improvement.

  11. Provide leadership and guidance to direct reports, fostering a culture of collaboration, innovation, and continuous improvement.

  12. Stay informed about developments in diabetes prevention, education, and billing, incorporating best practices into program design and delivery.

  13. All other duties as assigned.

WORKING CONDITIONS

There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature and air conditions.

MINIMUM REQUIREMENTS

EDUCATION REQUIREMENTS

Requires a Bachelor's degree in Behavioral or Social Science, Humanities, Business Administration, Public Administration or a closely related field.

EXPERIENCE REQUIREMENTS

Four years of experience in administration or professional work related to Urban Planning, Public Administration or Service Delivery in a social service agency, public department or a similar facility are required.

Directly related professional experience may be substituted for the education requirement on a year-for-year basis.

PREFERENCES

  • Masters degree in Public Health, Public Administration, or Behavioral Science

  • Bilingual in Spanish, Vietnamese, or Chinese

  • Familiarity with chronic disease/diabetes self-management programming or public health practices

  • Advance proficiency in data reporting and management using Microsoft Excel, EPIC, Qualtrics and other types of data software

  • Experience using Microsoft Office products including Word, Excel, PowerPoint, Teams
