Queen's Health System Temporary Patient Community Navigator - Queen's Clinically Integrated Physician Network (Full-Time, 40, Day) in Honolulu, Hawaii
RESPONSIBILITIES
This is a Temporary grant funded position.
I. JOB SUMMARY/RESPONSIBILITIES:
• Supports community health by establishing and maintaining trusting relationships with patients, families, and providers to promote health, recovery, resiliency, and wellness.
• Performs activities off-site, in the community, and makes patient home visits as required.
• Performs duties and responsibilities in accordance with policies and procedures and consistent with the goals and objective of the department and The Queen's Medical Center (QMC).
II. TYPICAL PHYSICAL DEMANDS:
• Essential: stooping/bending, finger dexterity, seeing, hearing, speaking; lifting usual weight of 10 pounds up to 25 pounds.
• Frequent: standing, walking, reaching at and below shoulder level.
• Occasional: sitting, kneeling, climbing stairs, walking on uneven ground, squatting, pushing/pulling usual weight of 10 pounds up to 25 pounds; carrying usual weight of 5 pounds; reaching above shoulder level.
• Operates computer, calculator, copier, facsimile, and telephone.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Must be willing to work both at the QMC and in the community. Work involves extensive off-site activities, including home and community-based visits to patients.
• Must have reliable transportation in order to make visits to sites where patients reside.
• May be exposed to negative responses from patient or family.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• High School diploma or equivalent.
• Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.
• Driver's abstract or evidence of good driving record.
• Meet and comply with qualifications set forth in QMC Fleet Safety Policy.
• Medical Assistant, Community Health Worker or Patient Navigation certification preferred.
• Current certification in BLS/HCP required.
B. EXPERIENCE:
• Two (2) years of relevant healthcare/social services experience; or one (1) year of healthcare/social services experience with a Community Health Worker certificate.
• Previous experience working with individuals with behavioral health and/or substance use issues preferred.
• Prior experience working with the underserved preferred.
