Responsibilities

MAJOR DUTIES:

  1. Clinical/Patient Care: Provides specialized, complex nursing care to adult outpatients with various illnesses. Plans, provides, and evaluates comprehensive nursing care utilizing expertise in infusion nursing appropriate to identify and treat existing and potential patient problems. Able to multi-task and care for patient within the Rheumatology, Infectious Disease and Nephrology clinics. Formulates care plan based upon an assessment of each patients physical performance status, psychosocial functioning, cultural/ethnic background, spiritual/religious beliefs, and cognitive status. Reviews and communicates pre-treatment laboratory and diagnostic tests to physician. Applies and maintains patient dressings per Infection Control policies. Educates patient and/or family members about their disease, treatments, and potential side effects/complications. Establishes rapport with patients and family members to provide emotional support. Makes and receives phone calls from patients for symptom monitoring and follow-up lab testing. Assist with 24 hour Automated Blood Pressure Monitoring. Counsels patients about available resources related to specific illness and initiates appropriate referrals. Administers prescribed medications after appropriate patient assessments. Performs venipuncture for blood sampling, inserts intravenous catheters, and access dialysis catheters and fistulas. Perform hemodialysis. Documents care in appropriate records. In the absence of a physician, independently determines and initiates appropriate emergency measures after identifying and interpreting life-threatening conditions, such as basic cardiopulmonary resuscitation and administration of oxygen. Assesses and monitors patient condition. Recognizes and interprets critical situations, and notifies physician or takes preplanned emergency measures until physician is available. Maintains nursing daily schedules. Creates, updates and maintains clinic Standard Operating Procedures (SOPs).

a. Administers and/or educates patient/family members to administer medications of therapy. Administer routine and complex drugs such as biological response modifiers, monoclonal antibody therapy, immunoglobulins, antibiotics, and erythropoietin stimulating agents (including iron) in accordance with prescribed protocols using various routes including oral, intravenous, intramuscular, subcutaneous, and intradermal. Assist physician with joint/trigger point injections. Continuously monitors response to therapy by routinely checking laboratory results and observing patients for adverse effects, particularly hypersensitivity reactions prior to and during treatment. Observes patient for signs and symptoms of toxicity or adverse reactions. Makes prompt, appropriate responses and adjusts infusion rates quickly according to rapid changing patient conditions related to newer therapeutic agents. Takes frequent vital signs and pulse oximetry measurements as indicated.

b. Maintenance of Venous Access Devices. Inserts peripheral venous lines for administration of medications and access hemodialysis catheters and fistulas.. Proficient with accessing various access lines, including but not limited to port-a-caths, PICCs, Hickman catheters, and Groshong catheters for medication administration and blood sampling. Administers fibrinolytic agents per protocol to restore patency of central venous catheters obstructed by clotted blood or fibrin sheath.

  1. Collaboration/Communication: Recommends and makes appropriate consults to chaplain, physical therapy, social work, clinical nurse specialists, and home care nursing. Communicates and collaborates with other staff members, health care providers, and inpatient units to maintain continuity of care. Takes phone calls from patients and adequately triages symptoms and patient concerns. Makes follow-up phone calls to patients to monitor symptom control after treatments. Supports and assists in coordination of nursing activities with allied departments, services and agencies. Supports aims and objectives of Departments of Nursing and Medicine. (10%)

  2. Research Support/Assistance: Familiar with current infusion literature and implements applicable research findings. Assists in monitoring and documenting nursing care activities for unit's performance improvement plan. Prepares reports, and enters workload/billing data into computer daily.

  3. Education: Assists with training nurses and students to care for patients requiring infusion therapy and/or other medical procedures within the Nephrology, Rheumatology and Infectious disease clinics. Provides guidance and instruction in administration to other nursing staff. Contributor to mandatory training requirements. Attends continuing education programs and in-services in accordance with unit and Department of Nursing guidelines. Educates patient and/or family members about the follow topics: disease process, pain and symptom control, medications and their side effects, coping with illness and treatments, maintenance of central line and venous access devices, precautions, nutrition, exercise, use of infusion pumps, proper techniques of self-medication and self-care, advance directives, and rehabilitation measures.

Qualifications

Must be a United States Citizen

Must possess a valid Registered Nurse License that has not elapsed/expired.

Must have an Associates, Bachelors or Master’s Degree in Nursing.This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
