Fairview Health Services Partial Hospitalization Program Intake Coordinator in Hibbing, Minnesota
Fairview Range is hiring an Partial Hospitalization Program Intake Coordinator for our Behavorial Health department in Hibbing, MN .
As an Intake Coordinator you will be responsible for maintaining the daily flow of patients within Behavioral Health service lines at Fairview Range, including coordinating the intake and referral process and assisting with discharge needs to the patients. The Intake Coordinator will primarily work with patients in the Partial Hospitalization Program but will serve as the main point of contact for patients in all BH service lines regarding coordination of services/appointments, referrals, and general service questions. This position will help streamline the process for patients within our Behavioral Health Service Lines under the direction and supervision of the Behavioral Health Director and Behavioral Health Service Line Social Worker Supervisor.
Schedule: This position will work 80 hours per two weeks. The shift length will be 8-hours on the day shift. Weekends: None

As an Intake Coordinator, you will be responsible for:
Coordinating and facilitating inquiry calls : Handle all inquiries regarding the Fairview Range BH service line programs.
Managing referral information : Collaborate with the PHP clinical team to coordinate all referral details.
Scheduling patients : Organize all patient appointments for the PHP program.
Managing patient and professional contacts : Handle reminder calls, service referrals, faxing releases, and discharge paperwork.
Completing the client intake process : Oversee the intake process for the Fairview Range Behavioral Health service line.
Consulting on unique referrals : Work with appropriate resources to address unique referrals or insurance/authorization discrepancies.
Monitoring schedules : Ensure optimal productivity by overseeing patient care providers' schedules.
Performing Referral Specialist tasks : Carry out Referral Specialist duties as needed for the Behavioral Health service line.
Required Qualifications
High school diploma or GED.
Prior experience in a secretarial, receptionist, or coordination role.
BCS certification current or obtain within 3 months of employment and maintains certification.
BLS current or obtain within 6 months of employment and maintains certification.
Preferred Qualifications
Completion of Administrative Assistant program for secretary/receptionist.
Experience working with scheduling behavioral health patients and providers.
