SUMMARY OF POSITION:
Responsible for coordinating the compassionate, efficient and effective care for people with serious illness who receive care in our palliative care clinics. Collaborate with referring providers, interdisciplinary palliative care team and community resources to address patient and caregiver needs in the ambulatory setting.
Position is full time, Monday through Friday, primarily in our interdisciplinary office at Hershey Medical Center.
MINIMUM QUALIFICATION(S):
Bachelor's degree in nursing required.
Three (3) years of nursing experience required.
Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact required.
