On an Average Day, you will:

  • Conduct comprehensive patient assessments to evaluate social, emotional, and environmental factors affecting patient care.

  • Provide clinical counseling and behavioral therapy to patients and families, ensuring adherence to HIPAA regulations.

  • Document medical records accurately and maintain confidentiality of patient information.

  • Collaborate with healthcare teams to develop and implement care plans tailored to individual patient needs.

  • Facilitate crisis intervention and provide support in hospice care settings, addressing the unique challenges faced by patients and their families.

What You Need:

  • Master’s degree or Doctoral degree in social work from a social work from a school of social work accredited by the Council of Social Work Education

  • Current state license as a Social Worker

  • 1 year of social work experience in a healthcare setting

  • Social work skills as defined as generally social work standards

  • 50% travel required

  • Valid driver’s license and insurance and reliable transportation to perform job tasks
