The Public Health Program Coordinator Family Health is a critical team member who assists with integrating research and best practices into programs for childbearing people, families with young children, and adolescent and young adult populations in Dakota County. The coordinator plays a vital role in the department s outreach and community engagement efforts and will collaborate with internal and external partners to meet our community s needs. This position is responsible for delegated program assessment, planning, implementation, and coordination of projects, as well as assisting with new employee orientation and staff training. Location & Schedule This hybrid position offers a mix of telework, in-person work at our Northern Service Center in West St. Paul, MN, and fieldwork throughout Dakota County. Work is performed Monday through Friday during regular business hours (8:00 a.m. to 4:30 p.m.). What you ll do As a Public Health Program Coordinator for Family Health, you will: Inform and guide evidence-based home visiting, surveillance, outreach, and case management. Assess community-wide target population health needs and resources. Initiate assigned policy and program planning, coordination, and implementation, integrating research and best practices to address complex systems and needs. Plan, implement, and evaluate projects, including continuous quality improvement and assurance initiatives. Develop and implement standardized documentation of assessment, interventions, and outcomes in an electronic health record. Work to advance inclusion, diversity, equity, and access in collaboration with colleagues and the community. Collaborate with internal and external partners to facilitate high-quality, holistic service delivery. Evaluate program outcomes, including interpretation and validation of statistical reports for ongoing planning, analysis, and establishment of service priorities. Develop and complete reports, plans, grant applications, forms, and communication tools as delegated. Provide direct public health services for small home visiting caseload. Performs public health emergency response functions as trained and assigned within the department s emergency response plan. Participate in the hiring process across Public Health and provides a lead role in orienting new public health staff, students, and other division employees to specific program goals and services. Participate in, facilitate, and plan meetings, as well as lead workgroups, and strategic planning initiatives for Family Health and departmental programs Coordinates and facilitates nursing student contracts, clinical placements and rotations across the department. Initiate assigned po icy and program planning, coordination, and implementation that address complex systems and needs. 3. Provide leadership and consultation to community agencies, health care providers, community groups, systems, and the public to assure at-risk population receives program services. 4. Evaluate program outcomes including interpretation and validation of statistical reports for ongoing planning, analysis, and establishment of service priorities. 5. Represent the department and county to public and internal and external workgroups and organizations, including developing and presenting educational programs and resources. 6. Performs public health emergency response functions as trained and assigned within the department's emergency response plan. 7. Participate in the hiring process and provide a lead role in orientating new public health staff, students, and other division employees to specific program goals and services. 8. Develop and complete reports, plans, grant applications, forms, and communication tools as delegated. 9. Provide direct public health services to families and communities with more complex needs 10. Participate and provide leadership on committees, workgroups, continuous quality improvement projects, and strategic planning initiatives. 11. Travel is required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to hazardous physical conditions such as environmental conditions of unsanitary dwellings and property, unsafe neighborhoods, challenging and potentially violent and unpredictable clients and family members, pets, and animals; exposure to atmospheric conditions such as chemicals, odors, dust, gases, poor ventilation, and cigarette smoke; exposure to hazardous materials such as chemicals, blood, and other body fluids; extreme temperatures; inadequate lighting; intense noise, and workspace restricting movement may occur on visits to client's homes. Equipment used may include but is not limited to a computer, printer, telephone, calculator, copy and fax machines, TV/VCR, video camera, nursing supplies and equipment including scales, syringes, hazardous waste, masks, and blood pressure equipment.
