The Community Health Navigator is responsible for providing outreach education, health assessments and community navigation services to identify, recruit, enroll and screen qualified program participants. The position is grant-funded through the Department of Public Health’s CT Early Detection & Prevention Program (CEDPP). The position is contingent upon continued funding. Responsibilities include but, are not limited to: · Provide health education & outreach activities to individuals in eligibility priority populations · Collaborate with local community agencies to identify potential underserved participants · Enroll eligible participants into the CT Early Detection & Prevention Program (CEDPP) · Assess eligible participants for barriers preventing participation in early detection screening and prevention services · Complete all required assessments, screenings and documentation · Work closely with medical providers and clinical personnel to ensure that clients have integrated, comprehensive, coordinated healthcare · Attend all required program meetings, DPH provider meetings and training sessions · Display and uphold HHC core values of dignity, compassion, service excellence, community and integrity. Consistently demonstrates caring for patients, for one another, and for the organization they are part of, and contributes to building trust, pride and camaraderie EDUCATION: · Minimum High School Diploma or equivalent, BS or BA in Social Services or related field strongly preferred EXPERIENCE * · 2-3 years of experience in a medical setting preferred. *KNOWLEDGE, SKILL REQUIREMENTS, AND DESIRED ATTRIBUTES · Bilingual English/Spanish highly preferred. · Excellent verbal and written communication skills a must. · Valid driver’s license and reliable transportation required. · Basic clerical and personal computer skills, including Microsoft Office suite are required. · Flexibility in work hours including some availability evenings and weekends when needed. · Ability to work independently and communicate effectively via phone, video conferencing and in person a must. · Ability to manage multiple priorities and ensure all grant program requirements are followed.
