Key Responsibilities:

  • Facilitating Patient Empowerment: Enhance communication and empower patients in their interactions with healthcare and social service systems to improve outcomes.

  • Barrier Troubleshooting: Identify and resolve barriers preventing patients from adhering to their care plans, ensuring they stay on track.

  • Community Outreach & Engagement: Actively engage high-risk patients through community outreach, fostering meaningful connections and support.

  • Health/Social Services Liaison: Serve as a bridge between health/social services and the community to improve access and culturally competent care delivery.

  • Care Management Development: Collaborate with primary care teams to develop care management strategies, including guidelines, disease management, and patient education for chronic disease self-management.

  • Building Community Health Capacity: Increase community and individual self-sufficiency by providing health education, advocacy, and social support to enhance overall well-being.

  • Collaborative Problem Solving: Work alongside patients and their care teams to find practical solutions to overcome healthcare barriers.

  • Resource Connection: Link patients with appropriate community resources and health center services for comprehensive care coordination.

  • Patient Follow-Up: Maintain consistent follow-up with patients through phone calls, home visits, and visits to alternative settings to ensure ongoing support.

  • General Support & Special Projects: Provide general support and contribute to special projects as needed, adapting to organizational priorities.

  • Public Education on Health Plans: Conduct educational initiatives to raise public awareness about the availability of Qualified Health Plans (QHPs).

  • Fair & Impartial Information Distribution: Offer clear, unbiased information to individuals seeking enrollment in QHPs, ensuring informed decisions.

  • Enrollment Assistance: Guide individuals through reviewing health coverage options and completing enrollment forms for QHPs.

  • Culturally Appropriate Information Delivery: Ensure all information provided is tailored to the cultural and linguistic needs of the population served by the marketplace.

  • Additional Duties: Perform any other tasks as assigned, contributing to the overall success of the organization.

Education and/or Experience Qualifications:

  • High School Diploma or equivalent required.

  • Bachelor’s degree from an accredited college or university in a related field preferred.

  • Two (2) years of experience working in community health, human services outreach, and/or healthcare preferred.

  • Desirable experience includes customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, and public speaking.

Additional Qualifications:

  • Knowledge and competencies with harm reduction, motivational interviewing, and relevant treatment modalities.

  • Familiarity with the culture and values of the local community.

  • Strong knowledge of principles and processes for providing customer and personal services.

  • Excellent verbal and written communication skills.

  • Proficient in Microsoft Excel.

  • Ability to actively listen, communicate effectively, and build rapport with clients.
