Position Summary: We are seeking a Licensed Practical Nurse (LPN) to provide exceptional care for our residents. You will deliver treatments, monitor health conditions, and collaborate with the care team to ensure optimal outcomes.

Key Responsibilities:

  • Administer medications and treatments as prescribed.

  • Monitor and document residents’ health and report changes to the RN or physician.

  • Collaborate on individualized care plans.

  • Supervise and guide nursing assistants to ensure quality care.

  • Ensure compliance with regulations and facility policies.

    Qualifications:

  • Active LPN license in NYS.

  • Experience in skilled nursing or long-term care preferred.

  • Strong communication and organizational skills.

  • Passion for delivering quality care.
