Position Summary: We are seeking a Licensed Practical Nurse (LPN) to provide exceptional care for our residents. You will deliver treatments, monitor health conditions, and collaborate with the care team to ensure optimal outcomes.
Key Responsibilities:
Administer medications and treatments as prescribed.
Monitor and document residents’ health and report changes to the RN or physician.
Collaborate on individualized care plans.
Supervise and guide nursing assistants to ensure quality care.
Ensure compliance with regulations and facility policies.
Qualifications:
Active LPN license in NYS.
Experience in skilled nursing or long-term care preferred.
Strong communication and organizational skills.
Passion for delivering quality care.
