Employment Opportunity: PRP Counselor (HYBRID) Trinity Supportive Housing and Consumer Servicesis seeking a dedicated and qualified PRP Counselor to join our team. This position involves providing essential support and community case management services to our clients. Position:PRP Counselor Type:Contractor Hours:TBD Location:On the road MARYLAND (Baltimore, Hagerstown, Silver Springs, Waldorf) Salary: $25-$30 hourly
Minimum Requirements:
- Education:
- Minimum of an associate degree in a public and/or mental health-related field.
- Alternatively, 30 hours or their equivalent of college credit in a public and/or mental health-related field.
- Experience:
- At least 2 years of work experience in a supervised public and/or mental health setting or its equivalent is preferred.
- Licensure:
- Must have a valid driver's license and be fully insured.
- Technical Requirements:
- Computer and adequate internet access.
Job Duties:
- Caseload Management:
- Manage a caseload of minor and/or adult clients.
- Client Visits:
- Conduct a minimum of 3 visits per month for minor clients and 6 visits per month for adult clients.
- Community Case Management:
- Provide community case management services.
- Intervention Services:
- Implement intervention services in support of the client's Individual Rehabilitation Plan (IRP). This may include navigating social services, housing resources and applications, and mobility application assistance.
- Documentation:
- Complete client progress notes within a 24-to-48-hour time frame after each client encounter in the EHR system ICANotes.
- Collaboration:
- Complete a monthly collaboration with the client's therapist and other collaborative organizations.
- Monthly Summary:
- Prepare a monthly summary for all clients within the required time frame.
- Meetings and Training:
- Attend the mandatory monthly All Staff Meeting and scheduled training sessions.
