The Impact You Can Make Team Impact Utilizing the nursing process, the centralized prescription refill nurse provides prescription related medication support to patients, including medication refills, prior authorization requests as directed by a provider order via electronic or telephone communication for established primary care patients with Glens Falls Medical Group Primary Care practices.* * * *The Glens Falls Hospital Impact Mission Our Mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. * * How You Will Fulfill Your Potential* * Responsibilities * Uses the nursing process to collect and document information in the electronic medical record (EMR) from patients requesting prescription refills for all primary care practices * Provides safe, ethical, direct and indirect patient care that is, patient centered and evidenced based * Participates in inbox management with other members of the care team for all primary care practices. * Communicates by telephone, email, instant messaging and via EMR message pools with patients, pharmacy staff, nurses and providers in a professional manner and documents all interactions in the EMR. * Reviews medication list and prepares orders for renewals as needed following established guidelines. * Completes medication prior authorizations following established protocols and documents status in the EMR. * Retrieve voicemails and processes prescription requests from messages left by patients requesting prescription renewals for all primary care practices. * Verifies and documents last office visit date, next office visit date and pain management contracts if applicable. Runs PMP query for narcotic prescriptions and document results of the query in the request to the provider. * Actively contributes to the professional development, morale and teamwork of staff while presenting a positive attitude and patient-minded vision, with patient satisfaction as the constant goal; uphold core values in all interactions. Education & Experience * Graduate of an accredited practical nursing program * 1 year of recent ambulatory care experience- preferably in primary care Licenses, Certifications, Registrations * Current unrestricted NYS LPN license Skills and Abilities * Microsoft Office application experience preferred * Epic EMR experience preferred Salary Range * The expected base rate for this Glens Falls, New York, United States-based positionis$28.00 to $35.06per hour.
