POSITION SUMMARY

Summary:
The Program Coordinator, Mental Health Services provides individual and family psychotherapy and functions as a core member of a collaborative care team. The coordinator is responsible for supporting mental health care and coordinating referrals to clinically indicated services outside the primary care clinic. This role provides wrap-around services to support clinician practices with supervision and case management. Services provided are, but not limited to: psychosocial assessment, consultation, crisis intervention, education, community liaison, information and referral, coordination of services, and advocacy.
Work Schedule:
Full-time position includes patient-facing clinical hours and hours for clinical follow-up and administrative duties including program development, supervision of team members, and community meetings.
SKILLS & ABILITIES
Education
Master's Degree in Mental Health Counseling, Social Work, or Marriage and Family Therapy required.
Experience
Two or more years related experience preferred.
Computer Skills
Proficient in Microsoft Office, email and basic computer skills for other software used throughout the hospital for your area of responsibility.
Must exhibit basic computer knowledge. Knowledge in using electronic medical records and other databases preferred.
Certificates & Licenses
Current with all state licensing requirements. Must have LCSW, or eligible to take exam and pass within 6 months of hire.
Other Requirements
Knowledge of behavioral health and addiction conditions and best practices for treatment and follow-up. Effective listening and interviewing skills to explore client needs and develop and monitor treatment plans. Excellent written and verbal communication skills and ability to document information in a timely and effective manner. Ability to work independently and to manage and prioritize tasks while also working as part of a collaborative team. Ability to build and maintain community partner relationships and establish professional collaboration with providers.
