Decatur County Memorial Hospital PROGRAM COORDINATOR, MENTAL HEALTH SERVICES in Greensburg, Indiana

  • POSITION SUMMARY

  • Practice Manager

  • Summary:

  • The Program Coordinator, Mental Health Services provides individual and family psychotherapy and functions as a core member of a collaborative care team. The coordinator is responsible for supporting mental health care and coordinating referrals to clinically indicated services outside the primary care clinic. This role provides wrap-around services to support clinician practices with supervision and case management. Services provided are, but not limited to: psychosocial assessment, consultation, crisis intervention, education, community liaison, information and referral, coordination of services, and advocacy.

  • Work Schedule:

  • Full-time position includes patient-facing clinical hours and hours for clinical follow-up and administrative duties including program development, supervision of team members, and community meetings.

  • SKILLS & ABILITIES

  • Education

  • Master's Degree in Mental Health Counseling, Social Work, or Marriage and Family Therapy required.

  • Experience

  • Two or more years related experience preferred.

  • Computer Skills

  • Proficient in Microsoft Office, email and basic computer skills for other software used throughout the hospital for your area of responsibility.

  • Must exhibit basic computer knowledge. Knowledge in using electronic medical records and other databases preferred.

  • Certificates & Licenses

  • Current with all state licensing requirements. Must have LCSW, or eligible to take exam and pass within 6 months of hire.

  • Other Requirements

  • Knowledge of behavioral health and addiction conditions and best practices for treatment and follow-up. Effective listening and interviewing skills to explore client needs and develop and monitor treatment plans. Excellent written and verbal communication skills and ability to document information in a timely and effective manner. Ability to work independently and to manage and prioritize tasks while also working as part of a collaborative team. Ability to build and maintain community partner relationships and establish professional collaboration with providers.
