Trinity Health Palliative Care RN Nurse Navigator in Grand Rapids, Michigan

Description:

The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcomes.

What the Nurse Navigator will do:

  • Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and

  • functional status in collaboration with multidisciplinary team members. Includes assessment of past and present treatment course.

  • Assists in organizing and coordinating timely patient appointments.

  • Understands and develops goals, treatment plan and provider options in concordance with evidence based standards and clinical trials.

  • Works directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care.

  • Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patients.

  • Uses critical thinking and problemsolving skills to analyze and synthesize data and formulate an appropriate case management

  • plan to address the patient’s individual and family needs.

What the Nurse Navigator will need:

  • Bachelor’s degree in Nursing required, Masters Degree in Nursing preferred.

  • Current State of Michigan Registered Nursing License, minimum. CPR or BCLS Certified.

  • Certified Nurse in sub-specialty (OCN, CNRN) and other sub-specialty certification, preferred.

  • Minimum: Three years of experience in an acute care setting. One year of experience in medical, surgical or radiation oncology (for oncology specialty).

  • Preferred: Case management experience or patient care leadership experience. Five years of experience in subspecialty.
