POSITION OVERVIEW:

This position is responsible for supporting parents and their children, preventing out-of-home placement or current placement disruption. Depending on assigned role, utilizes the Strengthening Families Protective Factors Framework and a variety of evidence-based practices to help families navigate the resources needed to become healthy, stable and safe.

This position is full time: Mon. - Fri., from 8:30 - 5:00, with occasional evenings and weekends.

The starting wage range is $21.00-$22.05 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Become trained and/or certified in assigned company Program, curriculum or Practice, and conduct duties in accordance to fidelity standards, grant requirements and program guidelines. Complete certification(s) required for position, which may include, but not be limited to High Fidelity Wraparound Standards, Credentials of the Colorado Standards for Peer and Family Specialist, Credentials of the Family Resource Center (FRC), Parents as Teachers (PaT), or other required training, as applicable to assigned role. Make initial contact, via Family Resource Center (FRC) walk-in or other internal or external referral source, with participant family to present available services and provide referrals and/or concrete support through one-on-one meetings, home visits, and/or phone contact. Upon enrollment, create a family working file with all necessary documentation; assist families in developing their individualized goal/plan. Administer all required assessments and screenings with families. Enter and maintain data as required by assigned program. Shares "lived" experience with families in accordance with fidelity model if required by role in the assigned program. Refer families to external services and resources when family needs are outside the scope of Navigator's professional training. While making a minimum of two contacts per month for families on caseload, create and maintain accurate and timely contact notes and enter timely data in appropriate program databases per program regulations. Collaborate with relevant agencies to assist clients with meeting goals. Plan, advertise, and implement lessons, activities and parenting classes based on specific program curriculum requirements. Accompany, facilitate, and/or attend family meetings/staffing times with other court, school, and/or legal related agencies. Provide or brainstorm ideas regarding transportation, as needed. Facilitate screenings for development, health, vision, and hearing to complete appropriate documentation and processes as needed. May share "lived" experience with participants/families by modeling effective personal interactions and behaviors, advocating for and supporting individuals, sharing personal experiences, mentoring individuals, and helping to build natural supports, if applicable.

GENERAL DUTIES AND RESPONSIBILITIES

Verbally translate for employees who are not bilingual in the event such services are needed. Develop and maintain up-to-date knowledge and information on programs and commun ty resources. Obtain necessary and ongoing training for services being provided. Qualifications
