REQUIREMENTS: Graduate of approved nursing program and hold a current State of Montana RN license. CPR Certification preferred or required within 6 months of hire. On-the-job training provided.DUTIES: Qualified applicant would provide assistances to Physicians as directed. Coordinates team-based care to provide health services to individuals, through effective partnerships with patients, their caregivers/families, community resources, and their physician. Provides diabetic education as instructed by the providers. Lead Chronic Care Management to support nursing staff and providers and attend and participate in ACO meetings.Full benefits package offered includes medical insurance with an option of free employee only health, dental and vision insurance. 401K with match after 6- months.
