Hunterdon Health Care System LSW Oncology Navigator in Flemington, New Jersey
Position Summary
Provide social work services for patients referred for social work assessment and intervention for newly diagnosed breast cancer patients at Hunterdon Breast Surgery Center and radiation oncology patients at Radiation Oncology at Hunterdon Regional Cancer Center. Provide surgical navigation from time of diagnosis of breast cancer through surgery by facilitating timely appointments during initial workup. Collaborates with other departments to problem-solve issues. Anticipate patient and caregiver needs, provide emotional support after diagnosis and provide resources as needed.
Primary Position Responsibilities
Assess and provide emotional support to patients and their families
Facilitate and coordinate patient centered care through their initial course of care through surgery
Serves as a patient and family advocate, providing education and addressing barriers to care.
Documentation of services provided in a patient’s electronic medical record
Facilitate and create documentation for Breast Cancer Case Conferences
Adheres to and actively participates with the Commission on Cancer and the National Accreditation Program for Breast Centers standards.
Participates in and contributes expertise to activities and functions that strengthen the cancer program (including, but not limited to, continuous program and quality improvement, relationship building, and community cancer awareness activities, fundraising events, etc.).
Minimum Education Requirements Required: Master’s Degree in Social Work, Psychology, Counseling or related field Completion of patient navigation training and credentialing
Minimum Years of Experience Required: 1-2 years graduate school or post-graduate social work experience in healthcare Preferred: Oncology experience
License, Registry or Certification Required: NJ Licensed Social Worker
Knowledge, Skills and/or Abilities Required: Excellent organization and communication skills. Self-directed, strong team player, collaborative, calm under stress, resourceful Preferred: Navigation experience
