Make a Difference
The Manager of Population Health & Post Acute Case Management provides direct leadership to assure that daily department operations function effectively and efficiently to meet the needs of providers and facility partners, patients and families, internal and external stakeholders and regulatory groups. This position ensures the appropriate clinical and performance outcomes are achieved and that effective and efficient team processes are maintained.
Exceptional Skills and Qualifications
Applicants for this position know how to collect and analyze data involving many variables to make decisions and convert to an action plan. Applicants can communicate complex/technical information effectively both spoken and in writing.
If a RN, an Associate’s Degree in nursing is required.
A Bachelor’s Degree in nursing is preferred.
Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) is required.
If a Social Worker, a Master’s Degree in Social Work is required.
Licensed as a Clinical Social Worker (LCSW) by the Indiana Professional Licensing Agency (IPLA) is required.
Three (3) or more years of experience in practice management design/leadership in one or more of the areas of specialty: CCRC, Long Term Care, Home Based Care, and Case Management is required.
Three (3) or more years of health plan experience with a provider perspective and strong orientation to value-based care principles and the health policy landscape is preferred.
Three (3) or more years of working knowledge of clinical quality metrics such as RAF, HEDIS, NCQA, or National Quality Forum type metrics.
Three (3) or more years of experience in data analytics comprehension.
Five (5) or more years of experience in provider relations, project management, quality assessment, and improvement.
