We are seeking an RN Case Manager to join our compassionate team at a well-established non-profit organization in the Northwest, dedicated to providing exceptional care for nearly a century.

Position Summary:

The RN Case Manager in Home Health is responsible for delivering and managing nursing care for patients in their homes according to physician prescriptions and in line with relevant laws, regulations, and agency policies. This role ensures comprehensive care coordination with other healthcare professionals involved in patient care, fostering continuity by sharing information with physicians, agency staff, patients, families, and community resources. The RN Case Manager operates in a Point of Care setting and reports directly to the Clinical Director.

Core Responsibilities:

• Administer high-quality nursing care to patients in their homes

• Coordinate and oversee the total care plan with healthcare team members

• Facilitate continuous patient care through effective communication and information sharing

• Comply with legal, regulatory, and agency requirements

• Maintain thorough patient records and documentation

• Participate in care planning meetings and case conferences

• Educate patients and their families on care needs and plans

• Perform regular patient evaluations and adjust care plans as needed

Qualifications:

• Active Registered Nurse (RN) license

• Experience in home health or a related healthcare field

• Excellent organizational and communication skills

• Ability to work both independently and collaboratively

• Compassionate, patient-centered care approach
