About the Program:
The RN will be a part of our Enhanced Services Facility. This small, 16-bed facility provides patients with a smooth transition from a hospital setting by providing assisted living and care for those with complex behavioral challenges. The main goal of this program is to help individuals participate and stabilize in their community.
General Responsibilities: As part of a multidisciplinary team, assess the physical health needs of residents; make appropriate referrals to community physicians; provide supervision, management, and administration of medication within the scope of practice

· Assessment and analysis. Initiates data collection and analysis that includes pertinent objective and subjective data regarding the health status of the residents.
