Hospital: Position is embedded in an AMG Pediatric or Family Provider's office
Location: Evansville, IN
The AMG PrimaryCare Provider Navigation Team is a multidisciplinary team made up of LPNs, RNs, and licensed Social Workers that performs case management (CM)) in the post acute environment.
Our team works with the primary care providers to help high risk patients manage their chronic and high risk conditions. Navigators follow up with patients post hospital stays to prevent readmissions, complete medication education, help patients find community resources, help with communication between the patient and the providers, and can refer a patient for behavioral health or Social Determinants of Health needs to our licensed social work team.
A Navigator will be responsible for a region and those providers in those offices. The navigator will be embedded in an office setting and this position does have an option of 1-2 days of remote work.

Responsibilities
Serve as a patient navigator throughout the continuum of care.
Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
Coordinate efforts for treatment and promotes quality improvement and educational efforts.
Compile data, track outcomes, and make recommendations for process improvement.
Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
Requirements
Licensure / Certification / Registration:
Licensed Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
Additional Preferences
RN and IND or Compact License
2-4 years Experience in pediatric population in inpatient, outpatient, or provider office preferred
Case Management experience a plus
Well versed in IT skills preferred
