WorkSource Oregon Community Health Worker in EUGENE, Oregon

Salary Range: \$20.50 - \$22.79 (This includes a potential phlebotomy differential) Salary Note: Bilingual candidates are eligible for an additional \$1.50 Bilingual Differential. This Bilingual Differential is not included in the salary range above. Position Summary: This position works closely with our clinical staff to improve access to medical treatment, behavioral health, lab services, medication and medication storage and other clinical care as needed treatment for target populations. This position primarily serves people who have Hepatitis C. The work will be primarily short-term assistance overcoming immediate barriers to care. The CHW assists the client in accessing medical programs, including Medicaid, Medicare, COBRA/Portability, private insurance, and Pharmaceutical Patient Assistance. They will also assist client in accessing private and government social services to meet basic needs such as housing, transportation and food, as well as alcohol and drug programs, mental health services, vocational programs and SSI/SSDI. Ongoing advocacy and support is provided to the client in order to maintain access to medical assistance programs and services, address barriers to care, coordinate referrals and minimize any break in access to services. Essential Functions and Responsibilities: Provide transportation and accompany clients to appointments Navigate clients to services and resources based on client need Provide direct one-on-one and phone care coordination services Make appropriate referrals, including follow up and documentation. Advocate with other case managers and other service providers on behalf of our clients Health education, including explanation of available services, and partner programs Monitoring of service delivery and reassessment of needs Maintain up to date information regarding other benefits and entitlements programs Maintain up-to-date and accurate client records Participate in required training pertaining to case management Participate in necessary meetings and teams Assist manager with all necessary reports Required Qualifications: TB test (provided at agency) Valid Oregon driver\'s license, or valid out of state driver\'s license with the ability to get an Oregon driver\'s license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy.
