UPMC Pharmacist in Erie, Pennsylvania

UPMC Hamot in Erie, PA welcomes you to join us as a full-time Pharmacist! Our role will serve variable days and evenings, 7:00a.m. - 3:30p.m. and 12:30p.m. - 9:00p.m.; our future teammate will work every third weekend and 2-3 holidays each year. Responsibilities:

  • Interpret physician orders and prescriptions to accurately prepare, calculate, and dispense medication dosages for both inpatient and outpatient settings, adhering to the Pennsylvania Pharmacy Practice Act and federal regulations.

  • Identify and promptly report suspected adverse drug reactions to ensure patient safety and improve therapeutic outcomes.

  • Sustain the medication formulary by minimizing non-formulary procurements, implementing therapeutic interchange protocols, and promoting rational drug therapy to optimize patient care.

  • Screen and maintain patient medication records for allergies, drug interactions, and therapeutic duplications, ensuring the accuracy of orders during verification processes.

  • Organize daily pharmacy operations and assist in supervising pharmacy technicians, aides, interns, and other personnel to enhance pharmaceutical care delivery.

  • Adhere to all regulatory requirements, professional standards, and hospital policies to ensure high-quality pharmaceutical care.

  • Communicate effectively with physicians and nurses to clarify unclear, illegible, or incomplete orders, fostering collaborative healthcare.

  • Provide drug information and educational programs to healthcare professionals and pharmacy students while leveraging advanced computer systems and technologies in medication management.

Required:

  • Graduate of an ACPE accredited College of Pharmacy

  • B.S in Pharmacy or Doctor of Pharmacy (PharmD) degree

  • Current licensure as a Pharmacist in practicing state

Preferred:

  • Pharmacist experience

  • Strong written and verbal communication skills

  • Excellent communication skills

Licensure, Certifications, and Clearances:

  • Pharmacist

  • Act 34
