HealthPartners Nurse Practitioner/Physician Assistant in Elk River, Minnesota

Certified Physician Assistant or Advance Practice RN (APRN) DATE: July 2024 DEPARTMENT: Family Practice REPORTS DIRECTLY TO: Clinic Manager and Clinic Medical Director POSITION PURPOSE: The Physician Assistant or APRN/Nurse Practitioner works within the three HealthPartners Riverway clinics located in Anoka, Elk River and Andover toprovide for the overall and ongoing medical responsibility for a patient's comprehensive care, encompassing preventative care and a full range of acute and chronic conditions including end-of-life as appropriate. This position provides support to the Clinician care team by doing in-basket and other support responsibilities as needed. The Physician Assistant/Nurse Practitioner works autonomously within their licensure and/or in collaboration with his/her supervising physician, in accordance with the Minnesota Board of Medical Practice and/or Minnesota Board of Nursing. REQUIRED PROFESSIONAL QUALIFICATIONS: Graduate of an accredited formal Physician Assistant or Family Nurse Practitioner program. Masters required. Current Minnesota PA/RN license. Current NCCPA or AANP certification. Approved or eligible for approval by the Minnesota Board of Medical Practice for prescription writing privileges. Ability to work independently. Demonstrates computer skills using Microsoft Office products or similar applications. Demonstrated excellent oral and written communication skills. Strong organizational skills Demonstrated problem solving skills. Experience with an automated medical record. Current BLS certification. One year of experience. Active member in professional association. PHYSICAL REQUIREMENTS No lifting requirements are identified but potential for lifting exists. Adequate vision, hearing, and tactile motor skills. Must be able to speak, read and write English proficiently. ACCOUNTABILITIES: Discriminate between situations requiring independent management, collaborative management with the Physician and immediate or further referral. Collaborate with Physician when unusual incidents or changes in patient's condition occur. Coordinate care involving other healthcare providers, outside vendors, or employers to meet the needs of the patient. Make appropriate referrals.
