Position Description:

Performs duties with higher levels of responsibility and complexity associated with a specialized function of a Community Health Worker. Duties may vary, but can be generally described as: planning, assigning, coordinating, directing, scheduling, evaluating, verifying, training, and processing of information. A thorough knowledge of policies, procedures, and methods for the area of responsibility is required. Initiative and independent judgments are frequently exercised in formulating procedures or methods, and in program coordination.

Major/Essential Functions:

  • Help develop, implement, and participate in strategic health education and outreach initiatives with staff, students, and other stakeholders, ensuring alignment with institutional goals and service agreements. This includes scheduling, facilitating communication across teams, tracking and reporting recruitment and education activities.

  • Support the development and implementation of tools to assess program performance and impact, collect data, gather feedback from participants, and provide support in the compiling of reports to ensure accountability and continuous improvement.

  • Complete program-required trainings on relevant health and outreach topics.

  • Attend meetings to report back to CHWs on outreach initiatives, work with CHWs to produce and/or update program materials.

  • Provide support in identifying grant opportunities and managing grant-funded projects to expand resources, improve program outreach, and support program sustainability.

  • Complete administrative tasks related to procurement, tracking program expenses, and ensuring compliance with funding agreements.

  • Continuously expand knowledge and understanding of health related topics including, community needs, resources, services and programs available. 8. Maintain confidentiality per HIPAA guidelines in regard to participant information.

  • Train community health workers (CHWs)/promotores in provision of health education/health information and effective community engagement strategies to ensure adherence to program protocols.

  • Under direction of program director, supervise and manage assignments of CHWs.

  • Adhere to all TTUHSCEP policies, procedures and processes.

  • Personally demonstrate, display, and act in accordance with TTUHSCEP’s values (Service, Respect, Accountability, Integrity, Advancement, and Teamwork). Actively promote and encourage staff across the institution.

  • Perform all other duties as assigned.

Preferred Qualifications:

  • Strong Communication Skills, Valid Drivers License with reliable transportation and willingness to travel within the community for outreach activities.

  • Flexibility to potentially work non-traditional hours as program needs dictate.

  • Bilingual (English/Spanish). Proficiency in Microsoft Office Suite.

  • Additional certifications or coursework in public health, community health, or related fields.

  • Prior experience in program coordination, supervision, or training CHWs.

  • Strong knowledge of health education principles and community outreach strategies.

  • In-depth understanding of the local community’s cultural, socioeconomic, and health-related challenges.

Required Qualifications:

  • High School diploma or equivalent.

  • Five years progressively responsible CHW experience.

  • Additional education and/or related experience to equal five years may substitute on a year for year basis.
