Mount Carmel Bluffs is seeking a Physical Therapist Assistant to join our team.

    ABOUT THIS COMMUNITY

    Mount Carmel Bluffs is a shared ministry of the Sisters of Charity, BVM and Presbyterian Homes & Services. This new senior community is in a beautiful and serene location atop the bluffs of Dubuque, Iowa, overlooking the Mississippi River.

    This community offers the following care options: Residential Care Facility (RCF), Residential Care Facility (RCF) with Memory Care, Long Term Care/ Care Center

    Address: 1160 Carmel Dr, Dubuque, IA 52003

    Near by towns include: Bellevue, Cascade, Peosta, Maquoketa, Platteville, WI, and Galena, IL.

Responsibilities

As a Physical Therapist Assistant you will be responsible to:

  • Provide treatment per the established plan of care and in accordance with physician orders.

  • Assist in identifying goals, prioritizing treatment, and selecting appropriate interventions.

  • Continuously assess changes in functional deficits and adjust treatment as appropriate.

  • Provide education to residents/clients, families, caregivers, and staff.

  • Collaborate with therapist on observations, progress towards goals, and recommended updates to plan of care and discharge.

  • Provide accurate and timely documentation & billing in electronic medical record.

  • Demonstrate understanding and knowledge of payer sources and regulatory requirements.

  • Coordinate with therapist on supervisory visits per regulations.

  • Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team .

Qualifications

  • Graduate for an accredited program for physical therapy assistants.

  • Current licensure in the state of practice as required

  • Competence in physical therapy treatment and documentation according to the Standards of Practice

  • Demonstrated compatibility with PHS's mission and operating philosophies.

  • Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
