Henry Ford Health System Post-Acute Care Placement Associate in Detroit, Michigan

The Post-Acute Care Placement Associate is a member of the Inpatient Case management Referral HUB team in the System Command Center. The member is responsible for collaborating with Case managers and Social workers to provide support by facilitating post-acute care facility placements and insurance authorization. Interfaces with insurance companies, community agencies and knowledge of electronic health record. Ability to interpret documents in the medical record.

EDUCATION AND EXPERIENCE:

High school diploma or G.E.D. required. Associate degree from an accredited academic institution preferred. One-year experience working in a health care or community setting required. Ability to assess, identify barriers and work in collaboration with case managers and other members of the care team to support patient¿s transitional needs. Must have thorough knowledge of (patient) medical record (EMR) components and releasing patient information. Must possess and demonstrate working knowledge and ability to work with computers and software programs including excel spreadsheets. Strong verbal communication and written documentation skills. Strong customer service and interpersonal skills with the ability to interact with all members of the health care community. Strong sense of compassion with the ability to successfully advocate for patients and their families. Proficiency with Microsoft Word, Excel and PowerPoint. Ability and willingness to self-motivate, self-manage and maintain flexibility in an ever changing environment.
