Description:

Abnormal Imaging: Birad 4 & 5, now what?

Initiate communication with patients upon learning they have a suspicious finding on their breast imaging exam.

Review imaging

Schedule patient for appropriate biopsy; educate and obtain medication and allergy lists

Fax biopsy order to referring provider with imaging report

Refer to Katzmann Breast Center

Imaging report recommended surgical excision

Patient unable to do outpatient biopsy

Biopsy results are finalized

Benign

Malignant

What biopsy completed in our facility or elsewhere? Enter results in Cerner (mammo tracking system)

Was the pathology positive for malignancy? Did they get referred to Katzmann or elsewhere?

ESSENTIAL FUNCTIONS:

· Educates and provides results as needed to breast imaging patients.

· Facilitates patient breast diagnostic imaging scheduling and access with a focus on the Accountable Care Organization, working closely with Mercy Clinic Health Coaches and Katzmann staff to provide guidance as patients transition between healthcare providers.

· Coordinates breast imaging follow up care working closely with referring physicians, radiologists, and breast surgeons. Navigates patients between screening, diagnostic imaging and biopsy to ensure timeliness of care.

· Identify strategies to make navigator referrals systematic for all breast patients

· Tracks patient populations to ensure compliance with follow up recommendations as appropriate. Assures consistent navigator care utilization of EHR tools.

· Assists with data management required for breast center of excellence specifications.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

· AMA approved two year training program in Radiologic Technology and registered by the ARRT.

· License to practice in the State of Iowa.

· Specialty certification required.

· Two years Mammography experience required.

· Management experience preferred.

· Obtains Breast Patient Navigator Certification within one year of hire, required

· Basic Life Support (BLS) for the Healthcare Provider certified or obtained within three (3) months of hire. Acceptable credentialing bodies and certifications include American Heart Association Basic Life Support for Healthcare Providers.

· Proof of completion of Mandatory Reporter abuse training specific to population serve within three (3) months of hire..
