Location: Denver Metro Area
Position: Licensed Practical Nurse (LPN)
Pay: $40-45 per visit
? Visit Duration: 30 minutes to 1 hour

LPN Responsibilities:
Patient Care Planning and Implementation: • Develop, execute, and evaluate personalized care plans • Adapt care strategies based on changing patient needs • Implement therapeutic, preventive, and rehabilitative nursing actions
Medical Care and Monitoring: • Administer prescribed medications and treatments • Monitor patient conditions and respond to changes promptly • Document care provided and treatment effectiveness
Collaboration and Communication: • Coordinate with physicians and healthcare team members • Participate in case conferences and team meetings • Report significant patient changes to appropriate medical professionals
Patient and Family Education: • Identify opportunities for health education and care coordination • Provide guidance on disease management and prevention • Assist with discharge planning and goal setting
Professional Standards Compliance: • Adhere to Nurse Practice Act and Standards of Nursing Practice • Follow state and local laws, and accreditation standards (e.g., ACHC, Joint Commission) • Uphold Patient Bill of Rights and Maxim Healthcare policies
Continuous Learning and Development: • Attend required educational in-services and training sessions • Stay updated on best practices in home health care • Contribute to improving care protocols and procedures
Quality Assurance: • Ensure all care meets or exceeds Maxim's high standards • Maintain accurate and timely documentation • Participate in quality improvement initiatives
Qualifications:
Current Colorado LPN license
Graduate of an accredited nursing school
Valid BLS certification
Current PPD or chest X-ray
1 year LPN experience preferred (new graduates welcome)
Strong communication and interpersonal skills
Reliable transportation
